Follow These 3 Steps to Submit a Claim

Step 1: Complete the Claim Form
- Download and complete the EasyPay Insurance Claim Form
- Download and complete the EasyPay Insurance Police Report Form
- Once completed, submit it along with all required documents:

Step 2: Get Confirmation
You’ll receive an SMS within 24 hours confirming that we’ve received your claim.

Step 3: Claim Processing
- If your documents are complete and valid, your claim will be finalised within 48 hours.
- If anything is missing, we’ll send you an SMS requesting the outstanding documents.
- You have up to 6 months to submit all required documents.
What Documents Do You Need?
Make sure to include the following with your claim form
- Completed and signed EasyPay Insurance claim form
- Certified ID copy of the deceased
- Certified ID copy of the nominated beneficiary
- Certified computerised death certificate
- Notice of death (BI1663)
- Claimant’s bank stamped statement/letter (not older than 3 months)
- Letter of authority (if no beneficiary is listed)
Additional Documents That May Be Required
If the death occurred in a rural area:
- DHA 1680 form, including the Chief/Headman’s contact details
If the death was due to an accident or unnatural causes:
- Completed EasyPay Police Report Form
- Motor Vehicle Accident report for vehicle-related deaths
If the claim is for a child:
- Unabridged birth certificate
- Guardianship letter from Social Development or proof of child support grant
- For stillborn claims: doctor’s letter, clinic card or discharge summary
If the claim is for a spouse or extended family member:
- Proof of relationship
Examples of acceptable proof for extended family:
- A signed letter from the dependent confirming their relationship to the deceased and their insurable interest
- Marriage certificate (e.g. brother-in-law or sister-in-law)
- Unabridged birth certificate (e.g. sibling)
